Supporting Independence with Daily Living Assistance
At Brockton Home Health Agency, we are committed to helping individuals live with dignity, comfort, and independence. That’s why we proudly offer Adult Foster Care (AFC) and Group Adult Foster Care (GAFC)—two MassHealth-supported programs tailored to meet the unique needs of elderly and disabled individuals.
Adult Foster Care (AFC)
Our AFC program allows a family member or friend to provide 24-hour care in the comfort of their own home. This program supports eligible individuals who require assistance with daily living activities but prefer to remain in a familiar, family-centered environment. Caregivers receive a tax-free stipend, and each participant benefits from personalized care coordination and ongoing clinical oversight.
Group Adult Foster Care (GAFC)
We also offer GAFC—a supportive care option for individuals residing in community housing or assisted living facilities. This program helps participants maintain their independence while receiving daily assistance with Activities of Daily Living (ADLs) such as bathing, dressing, mobility, and meals. With regular nursing oversight and care management, we ensure each client’s health, safety, and well-being are continuously supported.

What We Offer

- Assistance with Activities of Daily Living (ADLs)

- Support in Community or Assisted Living Settings
- Collaboration with Social Workers, Nurses, and Support Staff
Who It's For
This program is ideal for individuals who:
- Are 22 years or older
- Live in community housing or assisted living
- Require daily help with at least one ADL
- Need ongoing care coordination and oversight
- Are seeking support in maintaining independence


Eligibility Requirements
To qualify participants must:
- Be enrolled in MassHealth Standard or CommonHealth
- Have a physician’s summary confirming medical need for GAFC
- Be clinically approved by the designated MassHealth screening agency
- Meet financial criteria as outlined by Supplemental Security Income (SSI-G) standards
